DENTAL CROWNS

DENTAL VENEERS:

A veneer is a thin, custom-made shell or layer made of a special material, normally composite or porcelain, placed over a tooth to improve the aesthetics of a smile and to protect the tooth’s surface from damage.

 

A composite veneer can be built-up in the mouth, or indirectly fabricated by a dental technician in a dental lab, and later bonded with a special resin to the tooth.

 

In contrast, a porcelain veneer can only be fabricated in the lab, to be placed later inside the patient's mouth.

At Clinic32 we use both methods. We have an excellent lab support that can make porcelain veneers and we also have dentists, who are cosmetic experts, that can make a composite veneer directly on the patient's mouth. Porcelain veneers can last up to 15-20 years, but of course, like natural teeth, it all depends on the patient's habits and are still susceptible to damage.

PROFESSIONAL TOOTH WHITENING:

Tooth whitening, also known as bleaching, is a treatment that allows lighten teeth and help to remove stains and discoloration. This is possible by simply removing surface stains caused by extrinsic factors, stainers such as tea, coffee, red wine and tobacco.

DENTAL COMPOSITES:

Dental composites or filled resins are types of synthetic resins used in dentistry as restorative material or adhesives.  It is a tooth-colored mixture used to restore decayed teeth and for cosmetic improvements of the smile.

Dental composite resins also have certain properties that will benefit patients according to the patient's cavity. It has a micro-mechanic property that makes it more effective for filling small cavities where amalgam fillings are not as effective and could therefore fall out.

FULL MOUTH REHABILITATION

The mouth has 32 teeth including the wisdom teeth, so a full mouth restoration implies restoring somewhere between 20 to 28 teeth.

Are there different types of mouth restoration?

 

The procedure chosen depends on each particular case.

 

A Full Mouth Restoration or Reconstruction gives a completely new smile to a patient. Some patients need different procedures to get these ultimate results. It can be a combination of implants, crowns, veneers, root canals, periodontal treatments, gum surgeries, sinus lifts, bone grafts and fillings.

 

Some other people only need veneers to get their smile restored, while others may need an All on 6 /Multiple implants to make this possible.

 

Alternatives to a Full Mouth Restoration

 

A Full Mouth Restoration implies the reconstruction of the whole mouth, but some people do not need all their teeth done in order to improve their smile or to be able to eat. For example, a patient may need to get all his molars back, but can keep his natural front teeth if they are fine.  So again, it depends on the assessment of each particular case.

 

After collecting information from x rays, pictures, CT Scans, clinical exams and patient’s history, our dentists can determine what dental treatment plan is needed and what other options are possible for each individual case.

 

How is the procedure and how long does it take?

There are two types of reconstructive procedures:

Dental Implants and its restoration:Since a Mouth Restoration entails using crowns, veneers, bridges, dental implants, prosthesis, braces, and at times, surgery, it requires planning and experience to deliver beautiful long lasting results.(So first, the patient must come for placement ofdental implants which would take about 1-2 days. 10 weeks to 6 months later, when the jaw is completely healed, he/she must come back, to get the restoration.This healing period is the minimum we ask our patients to wait before making the Full Mouth Restoration, however, some people may heal faster, so it is possible to take a CT Scan before the 6 months to find out if the restoration can be done sooner.

 

Cosmetic restoration:If the dental restoration is just for aesthetics, the patient comes only one time for two weeks to get crowns or veneers, and has no need to come back again.

 

In the case of the Dental Implants, which are the post-operation cares?

 

We give our patients post op instructions and a very thorough explanation on how to take care of everything and what to expect before-during-after their surgery, based always on each patient´s case individually.

 

General care instructions are:

Brush and floss constantly.

Avoid foods that might break a porcelain restoration (nutshells, ice, jawbreakers, caramel almonds, etc.)

Get a professional cleaning 2 times per year.

Get a set of x-rays taken once a year.

What are dental implants?

​

Dental Implant Procedure 

A minor surgery is required to place a dental implant,and once it is in place, a crown is attached to it in order to provide a highly realistic-looking and functional prosthetic tooth.

There are various protocols used for implant dentistry, depending on the case. These are the most common and the methods we use:

 

One-Stage Dental Implants

This method is highly successful if used on the right type of case. It depends on finding the ideal conditions after a thorough diagnostic. One-stage dental implantstend to be most successful on front teeth. Additionally, they are great for complete edentulous jaws, which can take four to six implantsand restored immediately. Note make that immediate implants and write about guided surgery

 

Two-Stage Dental Implants

We commonly use the Two-Stage Dental Implantmethod. On the first stage, we place the dental implants, and wait for the healing period where the body is able to create a strong junction between the titanium dental implantsand the bone structure, resulting in a long-lasting dentalrestoration. This natural process is called Osseo integration.

 

On the second stage, after the Osseo integrationhas occurred, we place the crown. This is important because if you place the crownimmediately after the surgery, chances are that the pressure of your bite might be in the abutment crownor newprosthetic tooth, preventing the Osseo integrationof the dental implantand resulting in its loss.

Types of Dental Implant Prosthesis:

There are also different types of dental implant Prosthesis depending on the needs of each patient, as follows:

All on two dental implants (Removable denture)

This type of implants can be used to replace a whole set of teeth or arch (top and bottom).  This means a replacement of 10 to 14 teeth for each arch.  In this case, the patient will need at least two dental implants for the lower arch and at least 4 implants for the upper arch to attach two dentures into those implants.

At Clinic32 we commonly recommend and use only the all on 4 or all on 6 dental implants.

All on 4 dental implants

The all-on-4 concept comes from placing only 4 implants that will support a whole denture (bridge). These four dental implants act like pillars to the denture that supports the prosthetic teeth.  This denture is formed by a metal frame called hybrid denture, and the teeth, which are made with an acrylic material.  We insert each tooth individually on the metal frame and we add the fake gum.  This way when you smile it will look as if it was part of your natural bone structure.

All on 6 dental implants

The difference between the all-on-4 implants and this one is the number of implants that are used. All-on-6 has six implants.  The decision on which method to use depends on the clinical opinion of the dental practice.

 

At Clinic32, we recommend the All-on-6 procedure because we believe that it provides better strength and stability, especially when you chew or bite.  Having 6 dental implants allow dispersing the biting pressure evenly onto your implants and through the jaw, resulting in a significant amount of added strength, stability and long lasting benefit.

 

Also esthetically, the all-on-6 dental implant looks more natural, because we use porcelain to make the teeth, which is the material that looks more similar to your natural teeth.

 

Additionally, your teeth forms part of the metal frame, meaning each teeth will have metal in the bottom and porcelain on top.  This is the closest to having natural teeth that a patient might get, and they are artistically custom elaborated by the lab technician so they will fit perfectly to your individual requirements.

 

Who can be a candidate for an all-on-4 and all-on-6 dental implants?

Patients who have missing teeth or teeth in bad shape, and need to restore them fully on either the lower or the upper arch or on both al the same time.

Patients who have enough bone to get the implants If not, first they will have to get a bone graft done.

Patients that have gotten their teeth extracted must wait 3-4 months before placing the implants. The bone and gums must heal before placing an implant.

If all the conditions are ideal, patients can get teeth extracted, bone grafts done and implants placed during the same surgery.

ROOT CANAL TREATMENT

A root canal is a treatment used to repair and save a tooth that is badly decayed or becomes infected. During a root canal procedure, the nerve and pulp are removed and the inside of the tooth is cleaned and sealed. After which, the tooth will be restored with a crown or an other suitable restoration to protect and restore the tooth to full function.The tooth will then continues to function like any other tooth.

 

The Pulp is important in the tooth’s growth and development. however, once a tooth is fully mature is can survive without the pulp because the tooth continues to get nourished by the tissues surrounding it.contemporary endodontics  mandate the use of high magnification inorder to achieve this we at Clinic32 use a dental microscope. 

 

THE SIGNS TO LOOK FOR :

Prolonged sensitivity to heat or cold.

Tenderness to touch and chewing.

Discoloration of the tooth, and swelling, drainage and tenderness in the nearby bone and gum tissues. 

Sometimes, however, there are no symptoms at all.

 

At Clinic32 the root canal treatment is often performed in one or two visits which is case sensitive.

Invisalign is a popular removable brace that uses clear aligners to straighten teeth. Without a trace of metal it gently moves teeth using a series of transparent plastic aligners. Your Invisalign aligners will be custom-made just for you, and each one will move your teeth little-by-little.

Because there are no metal wires or brackets, Invisalign is very discreet. Best of all, you can take your brace out to enjoy all the foods that would be difficult to eat with a fixed brace on. Just make sure you wear your brace for a minimum of 20 hours each day.

•It’s very inconspicuous

•Your clinician will be able to show you a digital representation of how your teeth should look once you finish your treatment.

The cons of Invisalign®

Self-ligating Braces

 

Self-ligating braces look very similar to the traditional fixed braces that orthodontists have used to straighten teeth for decades. However, they are now available with Clear or Ceramic brackets, making them far more discreet.

They also use clips instead of elastic bands to hold the brace’s wire in place. This creates less friction and makes it easier for you to brush around your brace and keep your teeth clean during your orthodontic treatment.

  •  Self-ligating braces can be used to effectively treat simple or complex problems

•They can also achieve precise results by giving your clinician complete control over the way your teeth move.

The cons of self-ligating braces

  • Clear self-ligating braces are not completely hidden

  • You will need to brush carefully around brackets and wires in order to keep your teeth clean.

 

Lingual braces

Lingual braces attach to the lingual (tongue) side of your teeth. No one can see them, but they’ll be hard at work straightening your teeth from the inside. Your lingual brace will look and function just like a fixed brace, using brackets and wires to gradually move your teeth.

 

In the case of lingual braces, though, the brackets are custom-made to fit the more irregular back surfaces of your teeth. If you’re looking for a discreet brace, this is the crème da la crème of ‘invisible’braces.

  • The can be used to treat the majority of problems.

  • It can take a few days for your tongue to adapt to your brace and it may rub at first

•Lingual braces require a high degree of skill and specialist materials, so they are usually more expensive than other braces.

 

Retainers

 

Strictly speaking, retainers aren’t braces. Instead of moving your teeth, their job is to hold them in their new position once your braces come off. They’re so important, though, that we think they’re always worth a mention.

Removable retainers consist of clear vacuum-formed mouth guards that snugly fit your teeth to hold them in position. Fixed retainers are thin stainless steel wires that are permanently attached behind your front teeth where they stop your teeth from moving 24 hours a day, seven days a week.

The pros of retainers

  • After a short period of wearing your removable retainers full-time, you’ll only need to wear them at night

  • Fixed retainers are very unobtrusive and you’ll quickly forget they’re there.

If you don’t wear your retainers your teeth could move and undo all your hard work.

GUM TREATMENT

Periodontists specialize in treating Gum disorders both surgical and non surgical.

 

Scaling and Root Planing

Scaling and root planing is a careful cleaning of the root surfaces to remove plaque and calculus [tartar] from deep periodontal pockets and to smooth the tooth root to remove bacterial toxins.

 

Gum Graft Surgery

Exposed tooth roots are the result of gum recession. Gum graft surgery will repair the defect and help to prevent additional recession and bone loss.

 

Gum grafts can be used to cover roots or develop gum tissue where absent due to excessive gingival recession. During gum graft surgery, your periodontist takes gum tissue from another site of your mouth(usually the Palate) or another donor source to cover the exposed root. This can be done for one tooth or several teeth to even your gum line and reduce sensitivity.

 

The Benefits of Gum Graft are:

A gum graft can reduce further recession and bone loss. In some cases, it can cover exposed roots to protect them from decay. This may reduce tooth sensitivity and improve esthetics of your smile. Whether you have a gum graft to improve function or esthetics, patients often receive the benefits of both: a beautiful new smile and improved periodontal health –your keys to smiling, eating and speaking with comfort and confidence.

 

Dental Crown Lengthening Procedure

Crown lengthening procedure is done to correct your Gummy Smile. Your teeth may actually be the proper lengths, but they're covered with too much gum tissue. To correct this, we perform a crown lengthening procedure.

 

During the dental crown lengthening procedure, excess gum and bone tissue is reshaped to expose more of the natural tooth. This can be done to one tooth, to even your gum line, or to several teeth to expose a natural, broad smile.

 

Dental crown lengthening may also be done to make a restorative or cosmetic dental procedure possible. Perhaps your tooth is decayed, broken below the gum line, or has insufficient tooth structure for a restoration, such as a crown or bridge. Crown lengthening adjusts the gum and bone level to expose more of the tooth so it can be restored.

 

Periodontal Pocket Reduction Procedures

Your bone and gum tissue should fit snugly around your teeth like a turtleneck around your neck. When you have periodontal disease, this supporting tissue and bone is destroyed, forming "pockets" around the teeth.

 

Over time, these pockets become deeper, providing a larger space for bacteria to live. As bacteria develop around the teeth, they can accumulate and advance under the gum tissue. These deep pockets collect even more bacteria, resulting in further bone and tissue loss. Eventually, if too much bone is lost, the teeth will need to be extracted.

 

During the pocket reduction procedure, we at clinic32 fold back the gum tissue and removes the disease-causing bacteria before securing the tissue into place. In some cases, irregular surfaces of the damaged bone are smoothed to limit areas where disease-causing bacteria can hide. This allows the gum tissue to better reattach to healthy bone.

 

Reducing pocket depth and eliminating existing bacteria are important to prevent damage caused by the progression of periodontal disease and to help you maintain a healthy smile. Eliminating bacteria alone may not be sufficient to prevent disease recurrence.

LASER DENTISTRY
The acronym LASER stands for Light Amplification by Stimulated Emission of Radiation. 

 

Soft Tissue Lasers

The soft tissue lasers use a light wavelength that hemoglobin and water absorb easily. Hemoglobin is the molecule found in blood, which makes soft tissue lasers ideal for gum work. Some soft tissue lasers are diode lasers, which is a type of continuous-wave laser.

These lasers are ideal for cutting into soft tissue and sealing the exposed blood vessels at the same time. This is the reason you don’t bleed very much during laser dentistry and why healing is quicker after laser dentistry. Soft tissue lasers are great for cosmetic procedures because you can begin to see results right away. Soft tissue lasers are used for:

  • Lengthening crowns.

  • Reshaping the gums to create a more pleasing smile.

  • Removing folds in oral soft tissues caused by dentures.

  • Removing inflammation with relation to oral ulcers

  • Disinfecting Root Canals
